DNA Analysis Forms

Orders for all services are currently placed online through your BioDesktop Account.

To create a BioDesktop account:

Go to: https://biodesktop.uvm.edu/perl/desktop.cgi and click on create an account. You will need to choose a user name. You will be emailed a password which you can easily change after you login to your account for the first time. The facility staff will need to know the user name you choose to link the data distribution to your account. Please stop by the facility and fill out a BioDesktop Information Sheet, or email mary.shane@uvm.edu the following information: your name, date, email address, the name of the Principal Investigator of the Lab, BioDesktop user name, and phone number.

To place an order:

  1. Login to your BioDesktop Account.
  2. Click on the arrow to the left of "DNA Facility" to display the contents of this folder.
  3. Click on DNA Depot to go to the DNA Depot page.
  4. Click on the arrow by the "Order Form" folder in the "Orders" menu to display the contents of this folder.
  5. Click on the service you are requesting.
  6. Fill out all information on the order form. Fields with an asterisk are required.
  7. Click Submit at the bottom of the form. Look for 'Online order sent' message at the top of the page. You will receive an email confirmation that your order has been placed.

To print your order form:

  1. Click on "My Orders" in the "Orders" Menu
  2. Click on the Service requested, such as "Sequencing".
  3. Click on the box to the left of the order form (or forms) you would like to print.
  4. Click on Export PDF in the Orders Menu.
